Cocoa flavanols and cardiovascular events in COSMOS
The prespecified primary outcome was not significantly reduced (HR 0.90, 95% CI 0.78–1.02). A secondary cardiovascular-death result favored cocoa, while myocardial infarction, stroke, revascularization, and all-cause mortality were not significantly reduced; that secondary signal needs confirmation and does not establish routine prevention benefit.

- 21,442 US adults: women aged 65 years or older and men aged 60 years or older, without major cardiovascular disease or recently diagnosed cancer
- Intervention
- Cocoa extract providing 500 mg flavanols daily, including 80 mg epicatechin, versus placebo
- Outcome
- Total cardiovascular events, cardiovascular death, myocardial infarction, stroke, revascularization, and all-cause mortality
